Registration Form

This form as been renewed for the 2012-2013 season. If you have already filled in and returned the old version of this document titled 2012-2013 then you do not need to complete this version.

The Carnmoney FDC Registration Form MUST be completed by every player each season. It is important that the club retains up to date information on each of its players in order to operate efficiently. A section of this form includes agreement of parental consent, which MUST be read and understood by all parents and guardians.

Sportswear Order Form

If your child requires any new sportswear, items can be ordered via this form. Please complete as appropriate and return to your coach when sportswear is required. For information on the items which are available, please see the Sportswear section of this website.

Code of Conduct for Players and Parents

This form is covered by the Registration Form, but available to view online.
